The Patient App That Actually Gets Opened: Document Wallet, Health Assessment, and the Future of Engagement

Why most hospital patient apps gather dust on the home screen, and the three features that turn them into something patients actually return to.

Why patient apps fail

Most hospital patient apps in India have the same retention shape: a spike around the first appointment, then a cliff. Patients install it because the front desk insists, open it once, and never again. The reason is that the app's content is purely transactional — book, pay, read report — and offers nothing between visits. The successful apps in any category share one trait: they give the user something to come back to, even when they don't urgently need it.

The Document Wallet

This is the killer feature we keep hearing in customer interviews. Every Indian patient is sitting on a chaos of paper reports, prescriptions, vaccination cards, insurance policies, and discharge letters spread across email, WhatsApp, and physical files. The Document Wallet is a private, encrypted vault on the patient's phone where they can upload, auto-tag, search, and share any of it. The hospital's only role is to push the documents it generates straight into the wallet — but the patient owns it forever, across hospitals. That changes the app from a hospital tool into a personal one.

Health Assessment

  • Four-minute questionnaire covering cardiac risk (Indian-specific scoring), diabetic risk (IDRS), mental wellbeing (PHQ-9 + GAD-7), and a lifestyle index.
  • Generates a personalised report with red / amber / green flags per dimension.
  • Ends with a concrete next step — book a package, see a specialist, follow a lifestyle plan — so it converts into an action, not just a number.
  • Auto-reminds at 365 days for the next assessment, which is the highest-converting outbound notification we measure.

Family profiles

One login, multiple records. Parents managing kids, adult children managing elderly parents. This is how Indian households actually use healthcare — but most apps force one phone, one record. We let a primary account hold profiles for spouse, children and dependents, with a caregiver mode that lets a designated family member act on someone's behalf with consent.

Reminders that don't feel like spam

Three calibration rules: (1) one push per day max per profile, (2) every reminder must offer a one-tap action (not just information), (3) silent hours from 10 PM to 7 AM by default. Reminder click-through went from 6% to 22% after we enforced these rules.

What we tracked

  • Day-30 retention rose from 11% to 38% after the wallet shipped.
  • Average sessions per month per active user: 4.1 (industry average: 1.6).
  • Health Assessment 365-day re-engagement: 49% of completers come back exactly when reminded.
  • Document Wallet uploads after 90 days: median 14 per active user.
